Our practice is exclusively concierge medicine

The membership fee for the concierge program services:

• Two thousand dollars ($2000) per year for individual/primary member which can be paid by 4 equal quarterly payments
• Additional immediate family (spouse and/or children under 18) can become member of the program with additional one thousand dollars ($1000) per year for each family member which can be paid by 4 equal quarterly payments

If you already started filling up patients forms and you are trying to go back and finish it, please do not press new patient sign up, it will create duplicate forms.
