Membership Fees*
The membership fee for the program services:
- Two thousand dollars ($2000) per year for individual/primary member which can be paid by 4 equal quarterly payments
- Additional immediate family (spouse and/or children under 18) can become member of the program with additional one thousand dollars ($1000) per year for each family member which can be paid by 4 equal quarterly payments
